Community Care and Support Service / Community Floating Support

The Community Care and Support Service is open to individuals over the age of 18 years known to the Community Mental Health Team, who have a mental health problem and who need support to maintain their day to day community living in their own home.

What we do

Community Care and Support Service (Antrim, Ballymena, Cookstown, Magherafelt), Floating Support Service (Newtownabbey)

The Community Care and Support Service is open to individuals over the age of 18 years known to the Community Mental Health Team, who have a mental health problem and who need support to maintain their day to day community living in their own home.

The service provides support, assistance and guidance, enabling the individual to carry out daily living activities whilst encouraging and supporting the person to live independently in the community.

Community Care and Support Service / Community Floating Support

What we do

The service offers one-to-one support in the client’s own home. Clients are supported to achieve their individually chosen goals as part of their own recovery support plan.

We facilitate a variety of activities, including WRAP courses, alternative therapies and healthy eating sessions. The support workers can support clients if they would like to become involved in MindWise’s volunteering opportunities.

We also offer practical support, including help with budgeting, assistance with shopping and welfare support.

Areas we cover

The service operates in Antrim, Ballymena, Cookstown, Magherafelt and Newtownabbey. Referrals are made via the Community Mental Health Team (Northern Trust area only) specific to the service location.

How we’re funded

The Community care and Support service (Antrim, Ballymena, Cookstown, Magherafelt) is funded by the Northern Health and Social Care Trust.

The Floating Support service (Newtownabbey) is funding through the Supporting people programme
